Republic Services in Bettendorf faces a fine of $4,500 after a 40-year-old employee was killed in a work-related incident.


A Bettendorf company faces a $4,500 penalty after an employee died in a  work-related incident in December.

Republic Services, 6449 Valley Drive, was cited by the Occupational Safety and Health Administration on Jan. 25. The company has contested the proposed financial penalty.

The company's diesel technician, Derek Dale Christensen, 40, of Bettendorf was killed Dec. 11.

"At Republic Services, we prioritize safety above all else," said Jim Olson, senior vice president of safety at Republic. "We are working cooperatively with OSHA to resolve this matter. We remain committed to providing a safe workplace for all of our employees."

According to OSHA documents, Christensen was servicing the hydraulic system of a front-loader collection truck when the dumpster lift arms began to move and pinned his body between the lift arms and the truck cab.

According to the citation and notification of penalty documents, the company's energy control procedures did not "clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy."

The company failed to properly identify the potential (gravity) energy source from the lift arms that needs to be isolated, OSHA said.

Emergency responders attempted life-saving measures, according to a news release from Bettendorf police, but Christensen died at the scene from his injuries.

OHSA manages two categories of violations: serious or other than serious, OHSA Administrator Jens Nissen said. Serious violations involve some kind of impairment of the body, he said.

According to its website, the company is an industry leader in U.S. recycling and non-hazardous solid waste.
